*Effective* learning from incidents means assembling the richest understanding of an event (origins, history, backdrop, difficulties) from MULTIPLE and sometimes differing perspectives, and representing this synthesis for the broadest possible audience inside an org.
This includes collecting (see @jeli_io) and collating initial sets of data...
...and using that as fuel for productive directions to take in interviewing folks who responded or are familiar with the areas involved.
Getting good at this work is challenging, but it can be learned and improved just like any skill!
