Yes, United States, there is actually a state with a 59.5% test positivity rate, 165 per 100,000 residents with new infections (the 2nd highest in the world and for the pandemic, as well for deaths) and no đŸ˜· mandate.

And its northern neighbor has the highest case per capita in the world and for the entire pandemic. A mask mandate was issued 10 PM Friday night.
If there was a national plan, which should have been in place 11 months ago, this wouldn't have happened. There would have been a SWAT team to amp up the testing, get such states under control long before this point. A rally w/ >500,000 people would never have been allowed.
