There's something I used to teach residents and students that was wrong. I think i didn't teach it completely wrong, but def without the subtlety and complexity it needed
It was something like, "Dr Pal, pt doesn't want me to write this down but its the entire reason for the visit!"
And i think I used to teach the doc to tell the patient they need to write it down to be able to provide the care that was being requested.
And I don't think i knew the subtlety and complexity necessary to deal with this problem IRL.
I now frequently ask before typing, "can i write this down in your chart yet, or should we just talk about it for now?"
Then, we can get into a discussion of what really really needs to be in a chart vs what can perhaps be less directly stated, if stated at all...and of course all of you can imagine scenarios where this might be important...
Privacy protections only go so far in providing protection to the patient...mostly they protect institutions from liability.
