my mental template for describing addiction goes like this:
level 1 = addicted, out of control
level 2 = addicted, under control
level 3 = not addicted, but will use again
level 4 = not addicted, will not use again
things which you've never tried before = no data

how can addiction be under vs out of control? I'm thinking of it as being "addiction" when it's a critical part of maintaining your daily routine, when you actively feel worse without it. "Under control" just means that this suffering doesn't compel you to restock.
whereas "out of control" means that your first priority upon running out is to get more
so, with that out of the way... it's survey time

how addicted are you to caffeine?
how addicted are you to alcohol?
how addicted are you to nicotine? again, this question only applies to people who have used it in the past
couple more--how addicted are you to marijuana?
how addicted are you to adderall or similar stimulants?
and finally--how addicted are you to heroin or similar opiates?
