A very important part of Diabetes management. Insulin, it’s timely use, busting myths around it and ‘convincing’ patients to take insulin. I will explain few points in the thread below #Type2Diabetes #Insulin #Diabetes #Type2 #Medtwitter (1) https://twitter.com/dr_dsandeep/status/1358713448759087104
Myth: insulin once started, needs to be continued lifelong.

May not always be true. Staring on time, with appropriate #lifestyle (diet, exercise) changes w use of oral agents, it’s possible to stop insulin in a subset of patients once glucose better (2) #Diabetes #MedTwitter https://twitter.com/baxirahul/status/1351794304285462528
Myth: Dr, Starting insulin means I am a failure.

Need to explain patient abt glucotoxicity, T2DM natural history & secondary sulfonylurea failure in simple terms. This is imp part of making patient/relatives understand why insulin is indicated &what will be d expected course (3)
Myth: insulin can cause kidney failure

Very common myth. No truth in this. Diabetes is an important cause of kidney failure, particularly when diabetes hasn’t been well controlled. Insulin when started timely aims to control glucose levels & prevent complications. It’s safe (4)
Myth: Insulin injections 💉 are painful and difficult

Modern pen 🖊 devices are very sophisticated and can be easily used by most patients. Newer needles are much less painful. I keep demo pens and devices to show to patients &they are surprised to see how they work #Insulin (5)
Other issues that need to be addressed

Cost of insulin and glucose monitoring

Insulin during work and travel

Elderly patients with eye issues (doses need to be injected correctly & need good vision, understanding) #Diabetes (6)
Having said that #Diabetes is a #lifestyle disease & we need to find out where patients are going wrong. Indian meals can be very high in carbohydrates. Patients may not be walking/ exercising adequately. Addressing these can help a lot (7)
I feel it’s important to spend time with the patient/ family, and explain them to the best of our abilities about the rationale behind the treatment options. Some patients may buy time, ok. Review again during next visit & if results don’t improve, likely patient will agree (8)
Trust and belief of patient in us is very important. Once that is achieved, it becomes easier to convince. Once complications set in, most patients will agree for the offered treatment. Aim of #diabetes management is PREVENTION of COMPLICATIONS (9)
