2- 'We don’t work for the CCG, the acute provider trust, in a GP practice or as a health visitor. We all work for the people of the place, wrapping our complex, frequently hard to navigate (let alone to access) services & support mechanisms around our communities.'
3- yes so often I hear "that's a different team /dept". It feels the current structure encourages passing the responsibility rather than seeing themselves as part of my wellbeing team! #TeamAnna
4- "By listening harder we will learn also about the stuff we do that actually doesn’t work." yes so often the pathway or support offered isn't right but that's the only option commissioned. Co-production with people & seeing us as whole lives not conditions essential.
5- I am trying to write one plan for my physical health, social care & wellbeing because they all interlink but the system currently siloes me & therefore spends money on things that are less effective. Let's hope this opportunity is taken.
