While abortion is permitted in Pakistan, most doctors are not sensitised to undertake them or to even know that it is legal -- and women don't even ask, such is the culture of secrecy and shame surrounding the practice.
Instead, women turn to illegal abortion providers - literal back alley outfits - who use a mix of pills and physical intervention to expel the fetus.
Perhaps paradoxically, Pakistan is considered to have the world's highest rate of abortions per women. That's because contraception isn't widely available, reliable and affordable; there's also little effort to help women be literate about contraception
