Apologies for inactivity on Twitter. Our #PlymouthCOVIDCollaborative has been hard at work. The task: vaccinate 80 carehomes, 1800 patients, 2700 staff, across 17 GP surgeries, in 5 #primarycarenetworks IN 10 DAYS. What follows is a brief chronology of events /1
Tues 5th Jan: I was asked to join amazing #Plymouth PCN COVID Collaborative team as #COVIDvaccination coordinator for #carehomes.
Thursday 7th Jan: 1st 400 doses of #astrazenecca arrive
Monday 11th Jan: 1st of 80 #carehomes to have AZ vaccines piloted in @PathfieldsMG PCN

Tuesday 12th Jan: 3 other #primarycarenetworks - Drake, Waterside, and Mayflower - also pilot. Our city works well together and, true to Plymouth form, our #vaccteams are a mixture of teams from #primarycare and #communityservices (courtesy of @livewellsw) -thanks Livewell

Wednesday 13th Jan: Letter from @NHSEngland: "YOU HAVE TEN DAYS TO VACCINATE ALL YOUR CARE HOMES. DEADLINE SUNDAY 24TH JAN." At the time we had 300 doses of vaccine 1730 patients & 77 carehomes to go /4

13th-14th Jan: Lots of phone calls & 7 key Outcomes (see Tweets 5-10):
1. More vaccine: to cover all #carehomes. Thanks @Dochayter and @DAllcorn
2. Carehome staff vaccination: Derriford Hospital ( @UHP_NHS)agree to vaccinate all 2600 CH staff (Thanks Sue Wilkins @AnnJamesNHS)/5

3. Coordination of care home staff vac appointments: 2600 people descending on hospital would be chaos. Plymouth City Council @plymouthcc phoned staff to coordinate hospital appointments and avoid surge. Massive thanks Craig Mcardle and PCC

4. Increased number of vacc teams: conversations ++ with colleagues #communityservices (thanks Michelle Fogg) & #primarycare, enabled us to raise enough staff to support 5 vaccination teams at any one time (thanks @livewell and Primary Care). /7
5. Vacc team support: CCG ( @NHSDevonCCG) provide us with a Pharmacy tech to re-stock our depot, prepare cool boxes, and quickly resupply 5 vacc teams so they can get back on the road. This massively improved efficiency (thanks CCG)

6. Logistics: Plymouth map with carehomes, colour coded by PCN.

7. Coordination with our amazing 80 carehomes: their heroic staff could see light at end of tunnel and worked really hard phoning families, speaking to patients, & coordinated the day so that vacc teams could be in and out and on the road to next home ASAP. /10
Outcome of these 7 points:
Fri 14th-Sat 23rd Jan: teams were vaccinating between 12-18 care homes (200-350) patients PER DAY. Some Stats:
1. 21st January- highest number of patients vaccinated (380)
2. 22nd January- highest number of care homes vaccinated. (17 in 1 day).

Plymouth Vaccination Collaborative leaderboard:
1. First prize for Vacc team with most care homes vaccinated in 1 day: Waterside PCN - 5 carehomes (did this 3 times!)

2. First prize for Vacc team with most patients vaccinated in 1 day: Sound PCN - 130 patients

Result: all older peoples care homes now completed and most carehomes for younger people with LD also done. The Plymouth CoVID Collaborative has achieved this IN 9 DAYS, one day ahead of NHSE deadline.
The teams are tired but happy and will see some rest this weekend. /13

Conclusion: Delivery at this scale needs a collaborative "Team of teams" across carehomes, primary care, community services, hospital, CCG, & City Council, working together with one shared goal.
