Many (though certainly not all) of these problems could be solved by better lettering. I can understand how publishers might not factor a letterer into their budget — it’s an expense unique to comics, and why not just rely on the already contracted artists to handle it themselves? But it makes a HUGE difference in the reading experience.
Some artists are great letters, but many aren’t. Similarly, some of the publisher’s design folks might get comics lettering, but many won’t. Somebody needs to be holding the line for quality.
