A few years ago, I started to adopt a "Decision Log" with every project I worked on. It has become an invaluable artifact to point to on when a decision was made, who was involved, and the rationale used to determine the outcome.
The basic template I use is:
- What did we decide?
- Who was involved in the decision?
- What are the significant consequences of the decision
- Why did we make this choice?
- What tripwires would cause us to revisit the decision?
- Links to references

I should add, this log isn't purely design-related. Instead it offers the entire team access to contribute and document when decisions are made and reference them if needed.
