I've said it before and I'll say it again...we don't need tons of magical innovation in healthcare..we need communication..case in point - I get an email today for a patient being discharged friday (yes, email before the day of d/c)!!
I am flagged on 3 potential issues to follow up of which I am capable to follow up all 3 but I am able to flag two major system issues that might limit our ability to monitor community labs, etc....email back to specialist tonight and an email back to me that we will address it
now here is the magic...its happening BEFORE the patient leaves the building. This is shared care, a common care plan and doing the right thing for the patient. This needs to be the standard, not the exception. Our patients deserve nothing less
