Sam was sick because he was going through the trials, Dean was worried about that and then they recieved a video where Kevin tells them that he is dead. Of course Dean feels responsible for both things and he is in pain

This was Dean at his lowest... the Mark was literally changing him into something he didn't want to be. He had just beaten up Cas and he was seeing him hurt in that mirror, he just couldn't handle all of that anymore
