Maintaining clinical practice as PhD or DNP/DPT tenure stream faculty is very difficult despite the essential need in #Nursing and probably PT too.
While I believe current clinical practice is essential for faculty on a tenure track - it is untenable.
This is why 👇

The infamous "three legged stool", the traditional model of tenure stream academia -teaching,research,service- does not include clinical practice
Clinical faculty are often separate from tenure stream faculty
Tenure stream #Nursing faculty are usually not in clinical sites

To fit the traditional model, I made the argument that my NP work in the clinic a day a week and in the summer was "service" in addition to usual service/leadership work at the university
This increased my workload and was not recognized as value-added to my faculty role

Faculty who have not practiced for years are often rely on text book scenarios rather than contemporary practice.
Clinically-active faculty with current expertise enhance teaching and simulation activities with real world experiences -> better teachers.
4/10
For PhDs, research questions often arise during real world clinical scenarios when current practices do not meet a patient need or innovative approaches solve a problem.
Case studies/new problems often arise during RW practice and may inform future studies.
5/10
For DNPs (and DPT), clinical practice stimulates implementation of new EBP, quality improvement activities and evaluation of patient outcomes and PROs.
6/10
Practice and research doctorates working together can only generate new evidence and create meaningful research and implementation projects with real world outcomes.
Not just a three-pub dissertation or one QI project.

We need a new model - the four-legged stool - in academia for health professions
And interprofessional education #IPEP
Nursing and other clinical professions (PT/OT) in health care can learn from the #medicalmodel and embrace clinical practice as part of academia

With my I was granted tenure, I was not able to create a new academic model.
We need junior faculty to advocate for a new model that is relevant, and senior faculty to support it.

Finally, the humility of clinical practice, the wisdom of my patients and their families, and the impact of healthcare teams grounds my world views on health, science and education for the next generation of #nurses.
