1/5 Really blown away by this @JAMAInternalMed paper comparing EHR use (specifically EPIC) in US to non-US from @robert_huckman @DBatesSafety @AJHolmgren @Cutler_econ et al

✍️ Notes 33% more time
📫 Messages 164% more messages
✅ Orders 123% more time
2/5 Note Writing and Orders take up the majority of EHR time

- Orders are likely due to dx association for billing
- But note writing is interesting because US notes have a ⬆️ ratio of automated/manual text ➡️ so we are spending more time writing less....Waste?
3/5 The enormous difference in In-basket Messaging is striking!!

- The majority of the difference comes from system and team messages (i.e. non-patient facing) ➡️ I would assume this is scheduling, coordination, billing ?'s and hand-offs....
4/5 And After-hours EHR use is 36% higher in the US which I would assume is because there was not enough time to finish documentation during clinic (despite spending more time on EHR) ➡️ clinician burnout
5/5 This study confirms what clinicians already know from experience

But 53% more time spent on the EHR does not lead to 53% better patient outcomes

And sure, its only 31 min per day... but over a year that adds up to 16 extra works days!!!

We need to clawback from the EHR
