To see the crudeness that results when proper titles of the ruling class are not honored, look at this 2017 WashPost article that viciously mocks PhDs who insist on being addressed as "Doctor." The context was "Dr. Gorka" but it has generalized derision:

Perhaps the most savage mockery came from @iamsambee, who announced that only insecure losers with shoddy intellects want to be publicly addressed as "Doctor."
It was deeply misogynistic, as one of her examples was an actual medical doctor: @DrJillStein. Shameful stuff here:
