The report found that Fort Hood's sexual assault and harassment program was “structurally weak and under-resourced,” as well as “ineffective, to the extent that there was a permissive environment for sexual assault and sexual harassment.”

Army Secretary Ryan McCarthy directed the suspension or firing of 14 Fort Hood leaders, including Maj. Gen. Scott Efflandt who was the acting Fort Hood commander at the time of Spc. Vanessa Guillén’s death.

The report revealed an environment that has pushed women stationed at Fort Hood “into survival mode” and leaders who have been asleep at the wheel, turning a blind eye to well-established problems within their ranks.
