Shorter duration to surgery leads not only to better cognitive outcome, but also better seizure control. This begs the question - for pathologies we know are frequently intractable to medicine (ie cortical malformations) should we do surgery prior to 2 ASM failure?
Early surgery means earlier withdrawal of medications (and subsequent impacts on cognition). Early medication withdrawal after surgery uncovers surgical failure earlier, but does not impact chance of control back on meds or ultimate outcome. So why wait??
