What if there was a function embedded into the EHR platform that informed clinicians about a patient’s socioeconomic risk factors during consultation?
Ex: patient’s address gives you info on proximity to food desert, or even a walkability score?

It’d be cool if the EHR/telehealth platform could also give you a range of talking points to guide your conversation with the patient based on their risk assessment / background collected.
It changes the patient-clinician dynamic to be more socially conscious.

And this goes for the nurses and social workers too — asking the right questions pre and post consultation to enhance the patient experience, not just focusing on whether they’re using the RPM software that you prescribed.
