Anti-mask MDs are modern day Andrew Wakefields.

These MDs are rare in real life, but outspoken on Twitter.

Despite the credential, they don't actually see patients.

They have simple motivations: they seek influence among the misinformed masses that crave an alternative reality
What's bizarre, they are often hydroxychloriquine loyalists too. They eagerly support a mildly dangerous (I know because I prescribe it) pharmaceutical rather than a simple fabric barrier.

Why this disconnect?

It's nakedly political.
I've met a couple of these anti-mask MDs in real life. One tried to sell me his ICO. Another is actually a chiropractor (but intro's himself as a physician).
This isn't to say I've always been right. I underestimated covid-19 early on. But it's a lesser sin to underestimate Covid in February than to deny it in November.
