After establishing how tough those Zoan types are, Oda uses Usopp — someone as human but more durable than Nami — to show the devastation Ulti’s head butt can cause to a normal human’s body. The duo never had a chance.
Nami’s plea points to the fact that she had already been struck by Ulti off panel. And she knows she can’t afford to take any more punishment given the sheer brutality with which Ulti’s delivers these head butts. This is one of her worst fears realized.
So, when Nami says she will die if she gets head butted again, it is not a ruse to get out of the fight. She is certain that she’s at death’s door. Ussop’s reaction and advice to her support this hypothesis.

There are two notable times when Nami thought she was facing certain death, both of which she was on the verge of tears. But in Wano her tears are flowing uncontrollably. Why is she so emotional? Obviously she’s in physical pain. But there’s more imo.
On Skypiea Eneru asks Nami if she has a future she desired. Holding Luffy’s hat close to her chest, she says she has ONE desirable future. And since Nami explicitly vows to always quench Luffy’s thirst for adventure on Weatheria, we can deduce that Luffy is part of that future.
So, assuming Luffy never get sick of adventuring in the future (why would he?) Nami has vowed to always be a part of of that future. After all, what greater treasure is there for a treasure obsessed girl to travel around the world on endless adventures?
So, while it’s fair to assume that Nami’s tears are the product of the physical pain she’s currently under (obviously), her excessive crying could be her coming to the realization that she might not be alive to see Luffy’s dream fulfilled, robbing her of the future she desires.
