There are various ways to do this. Congressional committees could investigate, the executive branch could dedicate personnel within DHS or DOJ, or international bodies like the UN could get involved. Either way, the fact-finding should carry the promise of real accountability
As @TomJawetz notes, "there's criminal liability, there’s civil liability, there’s administrative liability," and all should be considered to prevent the constant, rampant negligence and outright malice from ever taking place again
