30yr old M friend w/ severe R sided LB & radicular pain for approx 9/52. Unable to stand upright, lie supine/prone, limping due to pain. Leg worse than back. Insidious onset. Trying best to stay active but struggling. Only meds that help - difene & solpadol. Has seen X2
Physiotherapists. Basic mobility/ROM work for lumbar spine which he does & no change. Has been referred to neurosurgery as intensity of pain & function have not improved. Here is his scan. Left 2017, right this week.
What advice would you give this man who is otherwise very fit and healthy and eager to return to sport?
