So it was emergency duty when a child came with liver laceration grade 4, pelvic injury, degloving injury of thigh and superficial femoral artery thrombus. As child came after 14 hrs, the hopes were less that limb can be salvaged.
And as there were no ICU beds available, it was difficult we could take it. Somehow parents agreed for taking risk for bleed from liver (due to heparin) for an attempt of thrombectomy. Had a made shift bed in adult ICU.
I went in for case with CTVS surgeon, we did thrombectomy and fasciotomy. I literally didn't sleep a min that night. Then post op child has arrest, gets CPR and limb also gets discoloured. Inspite of that we didn't loose hope and kept pushing.
Finally on Post op day 14, the limb is healthy and recovered. The child extubated.
Lesson: "Even if it is a very small chance, we should give a shot." Had i given up and sent child to other centre the child may have lost the leg in delay.
Sometimes we have to fight logistics, fate and take risks but when we get such results, there is nothing more satisfying than it. #some4pedsurg
