IBCLC here: foremilk/hindmilk imbalance used to be the thinking, but that’s outdated.

The focus needs to be on frequent, efficient removal of milk; not limiting baby’s time at the breast (i.e., 10 mins a side); and getting help if signs of oversupply.

Is your baby struggling with discomfort?

1) Breast milkshake! Vigorous massage prior to feeding to homogenize your milk.

2) compressions + gentle massage during bfg to increase fat intake.

3) Allow baby to finish 1st side 1st
Check in with an IBCLC to make sure baby is removing milk and feeding properly. The IBCLC can also give you specific guidance for dealing with oversupply; don’t just jump into block feeding or other stuff you read about online.

Hospital-based LCs have very limited time & the focus is on those early days— it all changes once you get home and the milk comes in!
