I got into the office before the boss, every day. I rarely took sick days or vacations. I made myself uber valuable. Showing up is half the battle.
Warning: This can turn into a BAD thing if you *never* take time off. Moderation is key, but THIS WORKS.
When I wanted a promotion:
I started acting like I already got it.
I started doing the job before getting the job title.
I made it easy for the organization to promote me because I was already doing the job, and doing it well.

I told each boss how I felt, and they knew that they could count on me to provide honest and objective feedback, even if the answer was "You're crazy".
I moved companies five times in my 14-year-career. Each move boosted my salary by at least 15%. Nothing murders wealth like a dead-end job.
Don't be afraid to change jobs to get more money or a better position.
