Yesterday I had a PrEP patient’s chlamydia test come back positive more than 2 weeks after I sent it. And we have had about 5x as many specimens cancelled for unclear reasons by outside labs since our lab is now sending out due to resource constraints.
We know that STIs and HIV disproportionately affect adolescents, BIPOC and LGBT folks. The CDCs suggestions to decrease screening intervals or forgo testing of certain anatomical sites will only make these inequities worse.
If you are asked to ration there are ways to maximize body sites tested with fewer test kits. Some of us have learned these strategies that CDC isn’t discussing when practicing in resource limited settings and before CLIA licensing of extragenital sites
That tier 3 of CDC recs is syndromic management is terrifying. In resource limited settings (which we’re in now🤦🏻‍♀️but didn’t have to be) we’ve learned the limitations of this strategy. Reason #9,999 we should have been using full powers of the Defense Production Act/
