The relevant passage is here. "Sam" is very clearly having issues beyond "his masculinized brain". Heyer seems to claim later that Sam's displays of suffering are transgender b/c he suffered largely privately rather than openly & insecurely as "ROGD" profile girls do
For what it's worth, I have met many detrans women who fit this profile of gender distress (& its expression privately & publicly) as well as trans ppl with this profile for whom transition is clearly not serving them well...
This anecdote tells me far more about what pundits like Heying uncharitably believe about ROGD-profile dysphoria (and those girls they attribute it to) than what "real transgenderism" actually looks like.
P.S. I've never known a male person to develop a relationship like this with an older adult female and then communicate wordlessly using a demonstration of harm to their body in this way. It is however intimately familiar to me as someone who knew many female self-injurers.
Sam showed you b/c Sam knew you would understand, female to female, what it meant to be in such suffering to bear it quietly & then to end it by reaching for a solution so intimately destructive. Showing a man would have no effect- the communication requires mutual understanding.
