To facilitate further advancement of the guide ext. inflate a balloon in the distal portion of the vessel. In most cases pushing the guide ext will be possible. If not, pulling the balloon gently with counteractive push on the extension will lead to a deep seating of the device.
As you can see even a quite long 4 mm stent can now be delivered without any problems

After successful stent deployment it may be necessary to put the guide ext. into the stented segment. It's easier if you let the guide ext. "slide" over the stent ballon

After delivering the stent don't forget to remove the guide ext. before you deploy it

If the guide ext. collapses under the calcified vessel wall, making advancement of the stent impossible, you can dilate it, it works on most of the cases.
