They're genuinely terrified that if they express themselves in ANY way, no one will care, or that someone may even berate them for it.
Some have even come to believe that to talk about themselves in any way, no matter how simple, makes them a narcissist. The fear of drawing attention to themselves at the expense of others is so great that they feel they'd rather endure a lifetime of silence.
Sometimes this feeling is born from actual interactions with people, sometimes not.
