This November 9th will be the 4th anniversary of the #Sandilands tram crash, which killed 7 and injured 62. I committed at the time to ensure that all causes were properly investigated. The core problem: RAIB said fatigue risk management was “not a factor in the accident at Sandilands junction”, despite finding that the driver may have fallen asleep, fatigue was endemic among tram drivers, and there were failings in the operator's fatigue risk management system.
RAIB responded in detail, taking my letter point by point, but failing to address my evidence and arguments (except in one case, where it admitted that its report contained an error - of which more below). In the one instance they agreed they had made an error, they updated one of their Recommendations. I did not bother to answer to RAIB's obfuscatory letter. I trust the coroner to unravel all this, and have ensured my information is available. In my letter, I reject RAIB's responses, point by point, and pose some direct questions as to whether they did or did not review specific documents at the time of their investigations. I still believe their report is deeply flawed and sadly lets down the families of the victims.
