I’ve seen this clip trending online for some couple of days now and I think the safety aspect which led to this incident should’ve been averted if some measures were put in place which I will like to highlight.
1. LMRA - Last minute risk assessment should’ve been done by the pilot/captain
of the surfer boat because it’s obvious from the clip that the swells are disturbing the boat and thereby making her to roll, maybe a call to the mother vessel
to provide a better leeway would’ve

...helped in this situation or change the heading of the smaller boat by approaching with her bow.
2. Backpacks
- on no occasion should any personal be allowed to carry any bags
either big or small, that should’ve been sent accros with line from the mother vessel

3. Boat landing Officer/Able sea man - Maybe on the port side of the pilot boat is a boat landing Officer but he should be on the bow of the boat to receive the personal by guiding on when to either standby or drop to the vessel
when she’s on the crest of the swells.

4. PFD - And finally Personal floating devices, it’s not clear enough if the passengers was with any PFD but that I’m sure everyone knows it’s mandatory.

Best of luck to my colleagues who are all at their various sites.
