Samantha Jones refers to renal: a thread
Calls switchboard who call the tertiary centre switchboard who then bleep the renal reg. This chain of command leads to a response quicker than expected.
The renal reg informs her that referrals are to be emailed only. Phone calls are not acceptable.
She appropriately returns some sass to the renal reg.
Renal finally give in and allow this discussion to go ahead (but an email referral proforma MUST follow).
Gets frustrated when the renal reg keeps interrupting her when she’s trying to talk.
She feels like the person on the other end of the phone isn’t even allowing her to f**k up before giving her a hard time.
Renal suggest examining the patient ‘properly’ when she couldn’t tell them the exact JVP height in millimetres.
Renal ask for the fluid balance so she signals to the nurses for help. She forgot to bring this vital information to the phone before calling.
When the fluid balance consists of multiple iterations of “drank a brew” and “PU’d”.
She also cant locate drug chart 4 out of 5 and hence doesn’t know how much furosemide the patient is prescribed.
Renal suggest you can never have too much furosemide anyway.
When she thought she had stopped all hyperkalaemia culprit meds but renal remind her that the patient is still on LMWH prophylaxis and trimethoprim.
She agrees when the renal reg points out that the patient has had a low bicarbonate the whole admission and nothing has been done about it.
Renal reg: “Is the patient a candidate for RRT anyway?”

She gets ready to note down the 238 bloods that need to be requested as part of the “renal screen”.
Renal are not happy the patient hasn’t had a urine dip nor PCR yet, so she relays this to the nursing staff.
When the phone call ends but she appears to have aged 15 years in the process.
Anyway, renal agree to see the patient on their satellite DGH ward round that happens on alternate even-numbered Tuesdays.
When she tells her consultant the outcome of the referral and they’re a bit nonplussed about it.
